Our program specializes in serving persons who have a diagnosis of Cerebral Palsy (CP), but also provides services to clients with other associated developmental disabilities.

Clientele

We utilize an assessment of each person seeking enrollment in Plus Center services to determine how the program could best serve and benefit them.

For the assessment, please fill out the Assessment Questionnaire and bring it with you to your appointment. An Assessment Questionnaire can be mailed or emailed to you.

Please email Plus Center to set up an assessment and to get an Assessment Questionnaire.

Please bring all the requested information at the time of the assessment. Once the assessment documentation is completed, we can discuss the recommended program and service options.

All information will be kept strictly confidential and only used in assisting Plus Center to evaluate the best environments for our clients.

Enrollment

Our program specializes in serving persons who have a diagnosis of Cerebral Palsy (CP), but also provides services to clients with other associated developmental disabilities.

Clients we serve may be challenged in any or all of the following:

Motor impairment or disability
Movement disorders
Development delays
Hypertonia or hypotonia
Inability to fully control motor function, such as muscle control or coordination
Involuntary movements

Services Provided

Morning and/or afternoon sessions at our center (typically between 2-6 hours per day)
In-Home Family Support
Independent Living Skills Training
Specialist Services
Personal Support
